The following forms are available for the upcoming Faith Formation Program 2018-2019.
Information indicated on the form must be complete. Information left blank will delay the registration process until all information is listed.
Families must be registered parishioners of St. Bernadette for their child to participate in the program.
A copy of the child/youth baptismal certificate MUST accompany each registration form. IF the child/youth was baptized at St. Bernadette, the date of baptism must be indicated on the form.
Parents are required to volunteer for at least one of the volunteer options listed on the registration form. Scheduling is flexible. Some opportunities are “as needed” to help with a special activity or as an extra pair of hands in the classroom.Program fees are due upon registration.

Elementary Form:This program is for grades Kindergarten through 5th grade.
Intermediate Sacrament Prep Year 1:This program is for youth in middle school who have not received the sacraments of Baptism, First Reconciliation and or First Eucharist and have never been in a faith formation program before.
Intermediate Sacrament Prep Year 2:This program is for youth who have completed Intermediate Sacrament Prep Year 1 in 2017-2018 OR 5th grade.
EDGE: This program is for youth in middle school, grades 6, 7, & 8 and have received the sacraments of Baptism, First Reconciliation, First Eucharist.
High Sl youth, grades 9-12, who are just beginning to prepare for Confirmation. High School youth who have not received the sacraments of Baptism, First Reconciliation, and or First Eucharist will require additional time to prepare for the sacraments. The Director of chool Confirmation Year 1:This program is for high schooFaith Formation will meet with the youth and parents to outline the additional time needed to receive these sacraments.
High School Confirmation Year 2: This program is for high school youth, grades 10-12, who have completed Confirmation Year 1 program in 2017-2018 OR completed Year 1 at a neighboring parish. Proof of completing Year 1 MUST be provided by the neighboring parish.
